Please read these Terms of Use carefully before using the MindLeaf website or booking any sessions. By accessing this website or engaging with any services offered through it, you agree to be bound by the terms outlined below.
1. About These Terms
These Terms of Use govern your access to and use of the MindLeaf website (the "Site") and the online therapy services provided through it (the "Services"). The Services are provided by Hazel, a licensed therapist operating under the name MindLeaf ("I", "me", or "my").
If you do not agree with any part of these terms, please do not use this Site or book any sessions.
2. Nature of Services
MindLeaf offers individual online therapy sessions conducted via secure video conferencing. These sessions are intended for adults (18 years and older) seeking support with mental health concerns such as anxiety, stress, burnout, life transitions, grief, self-esteem, and relationship issues.
The services provided are therapeutic in nature but do not constitute a substitute for psychiatric care, medication management, or emergency intervention. I do not prescribe medication.
3. Eligibility
To use the Services, you must:
Be at least 18 years of age
Be legally capable of entering into a binding agreement
Not be in an acute psychiatric crisis requiring inpatient care
Have access to a stable internet connection and a private, secure location for sessions
By booking a session, you confirm that you meet the above criteria.
4. Therapeutic Relationship
A formal therapeutic relationship is only established once both parties have completed an intake process and agreed to a therapy contract. Browsing this website, submitting an inquiry form, or attending a free consultation call does not constitute the formation of a therapeutic relationship.
The therapeutic relationship is subject to its own separate agreement, which will be provided to you before your first paid session.
5. Session Booking, Cancellation & Rescheduling
Sessions must be booked in advance through the online booking system.
You may cancel or reschedule a session at no charge up to 24 hours before the scheduled time.
Cancellations made less than 24 hours in advance may be subject to a late cancellation fee as outlined in your therapy agreement.
I reserve the right to cancel or reschedule sessions in cases of illness, emergency, or circumstances beyond my control. You will be notified as promptly as possible.
6. Fees & Payment
Session fees are listed on the Site and are subject to change with reasonable notice. Payment is due at the time of booking unless otherwise agreed. Accepted payment methods are detailed on the booking page.
Sliding scale rates may be available at my discretion for clients with demonstrated financial need. These must be requested before booking and are subject to availability.
Refunds are not issued for completed sessions. Refunds for cancelled sessions are handled in accordance with the cancellation policy above.
7. Confidentiality
All information shared during sessions is treated as strictly confidential in accordance with applicable professional and legal standards.
Exceptions to confidentiality include, but may not be limited to:
Situations where there is a serious and imminent risk of harm to you or another identified person
Disclosure of abuse or neglect involving a child or vulnerable adult
A court order requiring disclosure
Any other situation mandated by applicable law
I will make every reasonable effort to inform you before breaching confidentiality, except where doing so would put you or another person at risk.
8. Technology & Platform
Sessions are conducted via a third-party video conferencing platform. While I take reasonable steps to use secure, encrypted platforms, I cannot guarantee the absolute security of any internet-based communication. You are responsible for ensuring that your own device and internet connection are secure and private during sessions.
Technical difficulties may occasionally affect session quality. In such cases, we will make reasonable efforts to reconnect or reschedule.
9. Limitations of Liability
To the fullest extent permitted by applicable law, MindLeaf and Hazel shall not be liable for:
Any indirect, incidental, or consequential damages arising from your use of the Site or Services
Technical failures, data breaches, or interruptions outside of my reasonable control
Decisions made by you based on information shared during sessions
Any harm arising from your failure to seek emergency or appropriate medical/psychiatric care
Nothing in these Terms limits my liability for death, personal injury caused by negligence, or any other liability that cannot be excluded by law.
10. Intellectual Property
All content on the MindLeaf website — including text, design, graphics, and branding — is the intellectual property of Hazel and may not be copied, reproduced, or distributed without prior written permission.
11. Third-Party Links
The Site may contain links to third-party websites for informational purposes. I am not responsible for the content, privacy practices, or accuracy of any third-party site. Visiting those links is at your own risk.
12. Changes to These Terms
I reserve the right to update or modify these Terms of Use at any time. Changes will be posted on this page with an updated date. Continued use of the Site or Services following any update constitutes your acceptance of the revised terms.
13. Governing Law
These Terms shall be governed by and construed in accordance with the laws of United States. Any disputes shall be subject to the exclusive jurisdiction of the courts of United States.
